Please describe the major activities you have been engaged in during this period. They must be listed in order of importance.

You are asked to describe the significant activities undertaken by you during the period covered by the appraisal. Your supervisor will then rate your performance on each of these activities.

Your initial task is to identify and describe the major work elements which make up your position. This takes considerable thought. Review the tasks on your Position Questionnaire. If they are too general, more specific activities you performed during the rating period should be developed from these more general tasks.

Combine related tasks into a single, meaningful activity where possible. Rank these activities in order of importance in relationship to meeting the objectives of your position.

5. 
You should usually have about three to eight statements covering your activities for the review period.
