Added -portrait, -height, -width, flags to HighlightDummy; made -combined the default.

Changed the black-pass halftone thresholding to match that of ImagerSample.Halftone - documents that ask for grays only should now come out the same with this context as with the black-only PrintColor context (assuming that the device parameters are set up the same).
